What is an internal pelvic floor muscle assessment? 

What is an internal pelvic floor muscle assessment? 

This is an assessment of your pelvic floor muscles' ability to contract, relax, and elongate. The assessment consists of the physical therapist observing your vulvar tissue and palpating with one finger externally first to assess for pain or tenderness. Your physical therapist may ask you to “Kegel” or squeeze like you are stopping the flow of urine, cough, and bear down which allows the therapist to understand how your musculature is currently functioning. The therapist may then place one finger inside the vaginal canal and palpate the three different layers of pelvic floor muscles. Depending upon your symptoms internal palpation may be suggested to be performed in the anal canal. Ultimately, an internal pelvic floor muscle assessment allows your therapist to assess the muscle tone, strength, endurance, motor control, and sensation which will lead to the development of your individualized treatment plan to help you achieve your goals!

As an out-of-network healthcare provider this means we are able to provide high-quality, one-on-one, holistic care at each and every visit. There are no surprises in our billing. All costs of services are expected to be paid for prior to receiving care. Services are designed to be affordable while providing the highest quality of care. We provide super bills that you can submit to insurance for reimbursement depending on your insurance plan. Insurance can be confusing. Please give us a call and we can help answer your specific questions!

A superbill is a document that outlines the treatment services a patient has received from a healthcare provider. Patients can use this document to file an out-of-network claim depending on their insurance plan to receive partial or full reimbursement for cash-pay physical therapy services. Salt & Light PT will provide you with a superbill at the end of each appointment or upon request.

North Carolina is a direct access state for physical therapy services. Salt and Light PT is a direct access provider and does NOT require a referral prior to treatment. ​If you are planning to file an out-of-network claim with your insurance, it is your responsibility to obtain any necessary referrals and/or pre-authorizations that your insurance company may require for claim submission.

Can I have a pelvic floor muscle assessment and/or treatment on my period? 

Yes, you can receive internal pelvic treatment and/or assessment during your menstrual cycle. Bleeding while on your menstrual cycle will not limit your pelvic rehab care in any way and we will make you as comfortable as possible! If you prefer not to have internal pelvic treatment or assessment while on your period, no problem we can still work together that day on a variety of other things!

No, sex should not be painful! Having a pelvic PT perform an assessment and treatment can provide you with freedom from pain, leave you feeling reconnected to your body, confident in your sexuality, and empowered through your sexual health!

No! Contrary to popular belief and previous generational messaging it is not “normal” to leak urine after having a baby…In fact, it is never normal to leak urine even a little bit! Leakage of urine is a sign of pelvic floor dysfunction, and a pelvic assessment can help you get to the root cause of why leaks are happening. 

Do I have to live with back pain or pelvic pain during pregnancy?

Mama please know that we can help you during pregnancy, you don’t have to live with pain simply because you are pregnant! Pelvic girdle pain is any pain felt between the posterior iliac crest and the gluteal fold including the SI joint and the pubic symphysis. Pelvic girdle pain can develop during pregnancy and/or during the postpartum period. A pelvic physical therapy assessment and treatment can help you thrive during your pregnancy and into postpartum!

Is an internal assessment necessary during pregnancy? 

Not necessarily, an orthopedic screen is often a great place to start! Physical therapy pelvic assessments and interventions for prenatal and peripartum patients will consist of external pelvic muscle assessments (visualization and palpation) and treatment approaches. Internal pelvic muscle assessments and intervention will be reserved for extenuating circumstances when the patient is unable to be successfully treated with conservative measures during the prenatal and immediate peripartum phase. If an internal assessment is warranted approval from your OB/GYN is needed. Out of an abundance of caution internal pelvic floor muscle assessments and intervention are generally not performed during the first trimester of pregnancy, with high-risk antepartum diagnosis, and immediately postpartum prior to medical clearance post delivery.
